Wyoming Advance Directive Form
The Wyoming Living Will is titled "Instructions for Health Care" and is included in an Advance Directive packet together with a form for naming a "Power of Attorney for Health Care" (a.k.a. Health Care Proxy) and a form for specifying Organ and Tissue Donation.

Your signature must be notarized OR two (2) witnesses must sign your Wyoming Advance Directive in order for it to be legal.
