The Living Will in West Virginia is titled "Health Care Directive" and is included in an Advance Directive packet along with a form for establishing a "Medical Power of Attorney" (a.k.a. Health Care Proxy).

Your signature and the signatures of two (2) witnesses must be notarized for your WV Advance Directive to be legitimate. Witnesses MUST BE at least 18 years of age and NOT related to you by blood or marriage.
