The North Dakota Living Will is referred to as "Health Care Instructions" and is included in an Advance Directive packet together with a form for naming a "Health Care Agent" (a.k.a. Health Care Proxy) and specifying Anatomical Donation.

You and two (2) witnesses are need to sign your ND Advance Directive in order for it to be legitimate.
