The Missouri Living Will is known as the "Health Care Choices Directive" and is included in an Advance Directive packet along with a form for designating a Health Care Proxy.

Your signature must be notarized, and two (2) witnesses must also sign your MO Advance Directive in order for it to be legal. Your witnesses CANNOT be related to you or have financial ties to your estate.
