In addition to the Arizona Living Will, an Advance Directive packet also includes a form for appointing a Health Care Proxy.
Your signature must be notarized OR one (1) witness must sign your Arizona Advance Directive in order for it to be legal. The witness must be at least 18 years old and CANNOT be a member of your immediate family (blood, adoption, or marriage), a person named in your Will to receive a portion of your estate, your representative, or a health care provider.
